Ice Arrow - Revision history

DarkMasterMan: Blizzard fixed the missile skill AR% bug at an unknown point in D2R, I have added a bug fix note and updated other formatting (warning colors) to reflect this. Other formatting consistency changes (removing S template from +% Attacking Rating since it's not a synergy bonus, increasing level table to 1-20, 21-40, 41-60 instead of 1-10, 11-20, 21-30, etc.).
