Freezing Arrow - Revision history

DarkMasterMan: Blizzard fixed the missile skill AR% bug at an unknown point in D2R, I have added a bug fix note and updated other formatting (warning colors) to reflect this. Multiple other formatting consistency changes (level 1-10 stats, 11-20, etc. changed to level 1-20 stats, 21-40, etc.), improper linking, fixed incorrect mana cost for D2R stats.
