Be careful, though, if you end up killing one of the flying ones over water - it may get 'dropped,' but you'll never see it because it got dropped in the water, where it can't be dropped. It's a small bug. You'll not be able to find another one in the same game, so make a new one if you want it right then and there. Only one of these quest rewards are allowed per character per difficulty.

The Eye can be found in the Spider Cavern, located in the Spider Forest; the Brain can be found in the third floor Flayer Dungeon, in the Flayer Jungle; the Heart can be found on the second level of the Sewers, which has entrances spread througout the three Bazaars. The Flail can be found by defeating the Council in the Travincal. One of the members drops it when they die.
