As the last surviving clan elder, he has been driven close to madness by his grief over the imminent failure of his clan to protect the Worldstone. Constantly ruminating on the myriad alternate courses the elders could have taken, he lives with the guilt of not dying with his peers.
 Nihlathak spawns on a raised platform in the Halls of Vaught beyond Anya's portal from Harrogath in Act 5. He gains a Unique bonus in Nightmare and another in Hell. He has no minions but he is surrounded by 20 Ice Spawn, and he can summon more. He can also whip them into bloodlust like an Overseer, and transform them into Frenzied Ice Spawn. Both are always Immune to Cold (150%) in Hell.
